• Conduct thorough psychosocial assessments for patients through interviews and thorough reviews of medical histories, observing environmental factors that may affect recovery.
• Provide compassionate counseling and emotional support to patients and their families, assisting them with the adjustments and challenges they face during and post-treatment.
• Collaborate closely with the interdisciplinary team to craft detailed, individualized treatment plans that comprehensively address both psychosocial needs and rehabilitation goals.
• Educate patients about community resources, including support groups, housing assistance programs, and vocational training opportunities to guide them towards sustainable recovery.
• Participate as an integral member of the discharge planning team, facilitating smooth transitions from the detox center to community settings or further care options.
• Document clinical interventions and maintain accurate records in accordance with legal and ethical guidelines, adhering to HIPAA regulations to protect patient confidentiality.

Our detox center caters specifically to individuals facing substance use disorders and co-occurring mental health conditions. Understanding the unique complexities of addiction is crucial, as many of our patients may also experience challenges related to trauma, depression, or anxiety. • A Master’s degree in Social Work from an accredited institution.
• Active state licensure as a Licensed Clinical Social Worker (LCSW) in Pennsylvania.
• Strong understanding of the social, emotional, and environmental factors affecting individuals in recovery, particularly those related to addiction.
• Excellent communication skills, both verbally and in writing, to effectively deliver assessments and counseling.
• Proven ability to work collaboratively within an interdisciplinary team.

• Prior experience in a rehabilitation or detox setting, working directly with individuals facing addiction.
• Familiarity with evidence-based treatment modalities and therapeutic interventions for substance use disorders.
• Knowledge of community resources available to support recovery post-treatment.
