This full-time position allows for remote work, providing flexibility in your schedule while maintaining a strong connection with clients and their families. You will primarily collaborate with healthcare teams and community resources to ensure effective care delivery. While standard working hours are Monday through Friday from 8:00 AM to 4:30 PM, there may be occasional evening or weekend responsibilities that arise based on client needs, necessitating flexibility and on-call availability.

• Conduct comprehensive assessments of clients' psychosocial needs by analyzing emotional, mental, and social factors to develop effective care strategies.
• Collaborate with clients and their families to create personalized care plans that address their unique goals, preferences, and challenges.
• Provide counseling and emotional support to seniors and their families, assisting them in navigating significant life transitions, such as health crises or relocation.
• Advocate for clients by ensuring their rights are upheld and that they have access to necessary community resources and services.
• Participate in interdisciplinary team meetings with healthcare professionals to discuss patient care and coordinate services for improved outcomes.
• Make regular visits to assess clients' well-being and living conditions, identifying any concerns or safety issues that need to be addressed.
• Facilitate communication and conflict resolution among family members involved in the care of senior clients.
• Educate clients and families on community resources, support groups, and programs that enhance social engagement and quality of life.
• Ensure accurate documentation of all client interactions, assessments, care plans, and progress notes, in compliance with organizational policies.
• Stay knowledgeable about current social work practices and ethical standards to provide the highest quality service.

This role specifically centers on senior clients and their families, who face a myriad of emotional and practical challenges associated with aging. You will engage with this demographic by offering support that empowers them to regain control over their mental health and navigate the complexities of their lived experiences.

• A Master’s degree in Social Work from an accredited institution is required.
• You must possess a current state licensure as a Licensed Clinical Social Worker (LCSW).
• A minimum of two years of relevant social work experience is essential, with a preference for experience in geriatric or community settings.
• Familiarity with social work principles and a thorough understanding of aging-related issues and available community resources.

• Experience working directly with elderly populations and understanding their unique psychological needs.
• Background in utilizing specific therapeutic modalities to enhance client engagement and treatment outcomes.
• Proven ability to work collaboratively within interdisciplinary teams, demonstrating effective communication and problem-solving skills.
