Work Environment

This remote position allows for flexibility while ensuring you remain closely integrated with our team and the clients we serve. You will engage with clients primarily through telehealth platforms, allowing you to provide critical support and interventions from the comfort of your own home. Key Responsibilities

• Provide direct clinical services to clients experiencing major mental illnesses and severe emotional disorders through both virtual and community-based interventions.
• Participate actively in team meetings to develop and advance community-based treatment goals, ensuring alignment and collaboration among all team members.
• Conduct comprehensive assessments and brief, evidence-based screenings to identify client needs, developing individualized care plans based on the findings.
• Implement integrated care plans that connect clients with necessary medical and community resources, facilitating effective communication among clients, families, and healthcare providers.
• Engage in crisis intervention and management, providing support and immediate solutions during challenging situations.
• Carry a designated caseload, involving regular check-ins and follow-ups to monitor client progress and adjust treatment plans accordingly.
• Document client interactions and write reports in a timely manner to maintain accurate records and comply with organizational protocols.
• Participate in continuous professional development opportunities, including committees aimed at improving quality of care and service delivery within the agency.

Patient Population / Client Focus

This role primarily focuses on individuals receiving hospice care, including those coping with grief, end-of-life transitions, and complex emotional responses to serious health conditions. The successful candidate will navigate sensitive conversations, providing compassionate support tailored to the unique experiences of clients and families facing significant challenges associated with terminal illnesses.

Qualifications

• Must hold a Bachelor’s Degree in Social Work or a related field from an accredited institution.
• Must possess a valid Licensed Clinical Social Worker (LCSW) designation.
• At least 2 years of relevant clinical experience, particularly within mental health and hospice care environments.
• Demonstrated ability to work with diverse populations in a culturally competent manner.
• Exhibit strong organizational skills and the ability to manage multiple caseloads effectively.

Preferred Experience

• Experience working in hospice or palliative care settings is highly desirable.
• Familiarity with telehealth practices and electronic health record systems.
• Training in evidence-based therapies and techniques related to grief counseling and mental health.

Compensation and Benefits

This position offers a competitive salary range of USD 70,000 to 85,000 annually, commensurate with experience.
