Bright Star Counseling is seeking a dedicated Licensed Master Social Worker (LMSW) to join our compassionate and dynamic team in Scranton, Pennsylvania. This full-time position offers the unique opportunity to work remotely while providing vital social support to seniors and their families. As a key member of our community health clinic, you will have the responsibility to design and implement individualized care plans that promote well-being among a vulnerable population.

The full-time work schedule is from Monday to Friday, 8:00 AM to 4:30 PM, with occasional evening or weekend hours based on client needs.

• Conduct comprehensive assessments of the psychosocial needs of seniors and their families, addressing emotional, mental, and social factors.
• Develop and implement tailored care plans in collaboration with clients and their families, ensuring alignment with their individual goals and preferences.
• Provide counseling services to assist seniors in navigating life transitions, such as moving into assisted living or coping with the loss of loved ones.
• Advocate for clients’ rights to access vital community resources, including healthcare programs and support groups.
• Collaborate with interdisciplinary teams, including healthcare professionals, to coordinate comprehensive care and monitor client progress.
• Make regular visits to clients' homes to evaluate their living situations, provide emotional support, and address any identified safety or health concerns.
• Educate clients and families regarding available resources and programs designed to enhance social engagement and overall wellness.
• Document all client interactions, assessments, and care plans accurately in electronic health record (EHR) systems to ensure effective communication among team members.
• Stay updated on current practices and regulations in social work, adhering to ethical standards in all interactions.

The primary focus of this position is to serve seniors, a demographic that often faces distinct challenges such as age-related health issues, mental health concerns, and social isolation. You will be an advocate and support system for this vulnerable population, helping them navigate the complexities of aging with dignity and respect.

• Master's degree in Social Work from an accredited institution.
• Current and valid state licensure as a Licensed Master Social Worker (LMSW) in Pennsylvania.
• Minimum of two years of relevant social work experience, preferably in community health or geriatric settings.
• Strong understanding of the issues impacting the aging population and knowledge of available community resources.
• Exceptional assessment, counseling, and problem-solving skills, with a high degree of empathy and compassion.
• Proficient in communication and interpersonal skills, able to work effectively within interdisciplinary teams.
• Familiarity with electronic health record (EHR) software and basic computer applications.

• Experience working within a community health clinic or similar environment supporting older adults.
• Background in advocacy and resource navigation for senior clients.
• Knowledge in mental health services related to aging adults.
• Experience facilitating support groups and workshops for seniors and their families.
