This full-time position offers the unique opportunity to work remotely while providing vital social support to seniors and their families. As a key member of our community health clinic, you will have the responsibility to design and implement individualized care plans that promote well-being among a vulnerable population.

Work Environment

This position operates within a remote framework, allowing for flexibility while remaining highly collaborative. You will connect with clients and families through virtual platforms and home visits when necessary, ensuring a human touch in our digital age. The full-time work schedule is from Monday to Friday, 8:00 AM to 4:30 PM, with occasional evening or weekend hours based on client needs.

Key Responsibilities
  • Conduct comprehensive assessments of the psychosocial needs of seniors and their families, addressing emotional, mental, and social factors.
  • Develop and implement tailored care plans in collaboration with clients and their families, ensuring alignment with their individual goals and preferences.
  • Provide counseling services to assist seniors in navigating life transitions, such as moving into assisted living or coping with the loss of loved ones.
  • Advocate for clients’ rights to access vital community resources, including healthcare programs and support groups.
