Job description

Job Title

Independently manage a panel of patients with acute and chronic health conditions with an emphasis on health promotion and disease prevention using evidence-based practice.

Develop and implement an individualized plan to achieve optimal population health and disease management.

Demonstrate leadership and works collaboratively with the interdisciplinary team.

Order and interpret labs, tests, and diagnostic studies.

Prescribe pharmacologic and non-pharmacologic interventions.

Initiate referrals and consults to specialty services.

Provide education to patients and their families.

Assess and adjust the plan of care as needed to achieve optimal patient outcomes.

Collaborate with the physicians, nurses and other disciplines to provide care.

Provide ongoing patient care and management of health problems.

Order appropriate diagnostic tests, further specifies in clinical privileges.

Evaluate patient for other health care needs and request consultation as indicated.

Teach and counsel patients and families in HBPC care, overall medical conditions, initiate the plan of care and intervene with most suited treatment modalities for the patient.

Prescribe medications, diet, supplies, and home health needs according to clinical privileges.

Note: A current, valid driver's license is required for this position.
