Director of Case Management – Northern Virginia

Salary: $120,000 – $130,000 + Quarterly Bonus

This is an outstanding leadership opportunity for an experienced Case Management professional ready to step into a Director-level role within an 80-bed inpatient rehabilitation hospital in the Northern Virginia market. Reporting directly to the CEO, this position offers high visibility and the ability to make a meaningful impact on patient flow, outcomes, and overall care coordination.

Position Overview
The Director of Case Management is responsible for leading all aspects of the department, including utilization review, discharge planning, and care coordination. This leader will drive operational efficiency, ensure regulatory compliance, and support the team in delivering high-quality, patient-centered care.

Leadership Scope

  • Direct oversight of 5 full-time Case Managers, 1 Case Management Assistant, and 6 PRN staff
  • Collaborative, hands-on leadership approach with the ability to step in and support the team as needed
  • Reports directly to the CEO

Key Responsibilities

  • Lead and manage day-to-day Case Management operations
  • Oversee utilization review and discharge planning processes
  • Ensure compliance with regulatory and accreditation standards
  • Partner with interdisciplinary teams to optimize patient outcomes and length of stay
  • Support staff development, performance management, and department growth
  • Maintain efficient patient throughput and resource utilization

Qualifications

  • Minimum of 3 years of Case Management leadership experience
  • Strong background in utilization review and discharge planning
  • Acute care hospital experience required; inpatient rehabilitation experience preferred
  • RN (BSN required) or Social Work background preferred (non-clinical candidates considered with strong experience)
  • Candidates with strong acute care Case Management experience seeking leadership growth are encouraged to apply
  • Demonstrated stability in career history (2+ years in roles preferred)
  • CCM or ACM certification required within the first year (employer-supported)

  • Competitive base salary: $120,000 – $130,000
