The Company: Our Client is a growing healthcare organization that specializes in working with the geriatric population. The Position: We are looking for a full-time Registered Nurse Case Manager(RN) to provide care to patients via their individual care plans. The client offers the following compensation package:
  • $84,000- $108,000+ Salary Based on Experience

Responsibilities:
  • This Position Does Not Require Direct/ Bedside Patient Care
  • Monday- Friday Hybrid Schedule 8:00am- 4:00pm
  • 3 Days in the Community, 2 Days at home
  • No Nights
  • No Weekends
  • Responsible For Helping Patients Set Up Services
  • See Patient in Their Home, Complete Admin in Your Home
  • No On Call Responsibilities

Requirements:
  • MA RN Licensed
  • 2+ Years of Experience as a Nurse
  • 2+ Years of Home Care Experience Preferred, But Not Required
  • CPR Certified
  • Works With Interdisciplinary Team
  • Build Strong Relationships With Participants
  • Must Have Great Communication Skills
