Certified Nursing Assistant (CNA)
No nights. No Weekends. Tuition Reimbursement for LPN/RN Path.
Join a compassionate and supportive hospice team dedicated to making a meaningful difference in the lives of patients and families. The Care Team Hospice is seeking a CNA to provide personal care and comfort-focused support to hospice patients in their homes and facility settings while working alongside our interdisciplinary team to ensure dignity, compassion, and quality of life throughout the hospice journey.
Schedule, Hours & Territory
• Standard operating hours are Monday through Friday, 8:00 AM to 4:30 PM
• Based out of our Mishawaka, IN office, serving patients throughout an approximately one-hour surrounding service area.

Key Responsibilities
• Provide personal care, hygiene, and comfort measures
• Observe and report changes in patient condition
• Support patients and families with compassion and respect
• Document care provided accurately and timely in the EMR HCHB (Homecare Homebase)
• Active CNA certification
• Hospice aide experience preferred
• Experience using HCHB (Homecare Homebase) preferred
Field Role Requirements
• Active driver's license
• Ability to pass a criminal background check
• Ability to pass a Motor Vehicle Record (MVR) check
• Current TB testing in accordance with agency policy
• Current CPR certification as required for the role
