Job Summary:
Fairport Baptist Homes Adult Care Facility is seeking a compassionate and detail-oriented UAS Certified Registered Nurse (RN) to join our assisted living team on a part-time basis. The UAS Registered Nurse (RN) role focuses on conducting Uniform Assessment System (UAS) evaluations for residents, supporting individualized care planning, Medicaid eligibility processes, and regulatory compliance. The ideal candidate has experience in Assisted Living Programs (ALP) and is committed to delivering high-quality, resident-centered care.

UAS Certified Registered Nurse (RN) Key Responsibilities:

  • Conduct timely and accurate UAS assessments for assisted living and ALP residents

  • Evaluate residents’ physical, cognitive, and psychosocial needs while supporting Medicaid documentation requirements

  • Collaborate with interdisciplinary teams to develop and maintain individualized service plans

  • Ensure compliance with New York State regulations, facility policies, and documentation standards

  • Communicate effectively with residents, families, and care team members while maintaining accurate electronic records

UAS Certified Registered Nurse (RN) Qualifications:

  • Active New York State Registered Nurse (RN) license and current UAS certification required

  • Experience with Assisted Living Programs (ALP), Medicaid processes, and community-based or long-term care settings preferred

  • Strong clinical assessment, documentation, and patient evaluation skills

  • Excellent communication, organizational, and time management abilities

  • Ability to work both independently and collaboratively within a healthcare team

Compensation & Schedule
