This role is primarily remote, allowing you the flexibility to create a balanced work-life dynamic. While you will frequently engage with patients virtually, you are part of a close-knit team that meets regularly for collaborative discussions and strategy sessions. The hospice center serves as a vital part of our integrated care approach, ensuring that even out-of-office clinicians remain connected and supported.

• Conduct thorough assessments and provide evidence-based psychotherapy to adult patients, tailoring approaches to their unique diagnoses and presenting challenges.
• Collaborate closely with multidisciplinary teams, including psychiatrists and other clinicians, to develop comprehensive care plans that address the complexities of each patient's condition.
• Document patient interactions, including detailed assessments, treatment plans, and therapy progress notes, in alignment with departmental policies to ensure high-quality patient care.
• Engage in regular consultations with other healthcare professionals to discuss patient progress and modify treatment plans as necessary.
• If interested, you can also take on the role of educator by teaching classes to medical students or supervising residents during their psychotherapy training.

Your work will focus predominantly on a diverse patient population, including older adults facing complex emotional, psychological, and social challenges. As a social worker in a hospice setting, you will play a critical role in addressing end-of-life issues, providing support and guidance to not just the patients, but also their families, ensuring a compassionate approach to care delivery.

• A Master's degree in Social Work from an accredited institution is required.
• A valid LCSW license in the state of California is mandatory for this position.
• Proven experience in clinical settings, particularly with older adults, is essential.
• Strong clinical assessment and diagnostic skills, along with the ability to apply evidence-based practices.
• Exceptional communication and interpersonal skills, capable of building rapport with patients and their families.
• An ability to work independently while thriving in a team-based environment.

• Experience in hospice, palliative, or geriatric care settings is highly desirable.
• Familiarity with telehealth tools and electronic health record systems will be advantageous.
• Background in providing psychoeducation to patients and families regarding mental health issues.
