Description
Social Worker needed for per diem role in Philadelphia County.
Provides clinical social work services across inpatient and outpatient settings, including case management for glaucoma research programs. Conducts psychosocial assessments, supports high-risk patients, develops discharge plans, addresses barriers to care, and coordinates community resources. Collaborates with interdisciplinary teams, documents in EMR/REDCAP, tracks research activity, and supports projects.
Position Requirements
PA LSW; strong communication, organization, and case management skills. Preferred: healthcare experience, work with older/underserved populations, 10+ years of experience, and interest in vision research.
