Location: Travel within Lewiston / Moscow, ID & Pullman, WA
Work Schedule: Full-time
Note this role is mostly in various senior living settings and may include varying levels of drive time during the day to these senior homes and communities. Job is 8am-5pm plus drive time prior to 8am to the first senior and drive time after 5pm from the last senior's home.

At the same time, the two resources we need most to address this problem - caregivers and primary care providers - are stretched thinner than ever.
Savoy aims to make aging better by restoring agency to seniors and integrating caregiving into the heart of healthcare to allow seniors to spend more quality days at home - and fewer days in the hospital. Today, this includes going into individual homes as well as partnering with senior living communities to coordinate and manage clinical gaps for these seniors, their caregivers, and the care ecosystem. Savoy deploys a team of clinical experts both in-person and virtually paired with proprietary technology to drive clinical outcomes - without disrupting the current care.
Position Overview:
Savoy Life is seeking a dedicated Resident Care Coordinator (RCC) to provide clinical and operations support to seniors in and around Lewiston / Moscow, ID & Pullman, WA. The Resident Care Coordinator will work closely with an assigned Savoy Life RN as well as well as a Savoy provider to provide healthcare to seniors in various settings including senior living (assisted living, independent living, memory care units) as well as individual homes with potential for other settings as well (such as adult family homes).
Key Responsibilities:
  • Care Delivery Back Office Support
    1. Schedule various patient visits in the EHR to optimize the RN and APP's time while coordinating with patient and caregiver needs
    2. Answer inbound calls, text messages, and voicemails in a professional and timely manner
    3. Monitor and manage inbound and outbound faxes
    4. Enter data, scan files, and print files as needed to support patient care and maintain updated historical records in the EHR in an organized and timely manner
    5. Identify and manage health plan insurance for residents
  • Care Coordination Support
    1. Support the RN and Advance Practice Provider (APP) by coordinating patient care to drive patient outcomes
    2. Work with the local Savoy team to collaborate with other community resources that aid in Savoy's goals of supporting patient care
    3. Manage facilitated telehealth visits with Savoy's virtual provider via a tablet
    4. Support the ongoing care with direct RCC-led resident visits
    5. Manage communications and foster positive relationships with patients as well as others involved including caregivers, families, primary care providers, specialists, pharmacies, and senior living community staff
    6. Drive engagement with eligible residents, POAs, and families through education on Savoy's offering to get consent forms signed

Qualifications:
  • High School Diploma / GED (or higher)
  • 2+ years of experience in healthcare with a focus on care coordination and healthcare administrative support
  • Medical Assistant, Patient Care Technician, or equivalent certification preferred, otherwise 5+ years of healthcare experience
  • Experience in geriatric care or senior living community is highly desirable
  • Must possess a valid and current driver's license & be comfortable driving around to different senior home settings including senior living communities as well as individual homes and beyond
  • Excellent electronic medical record (EMR) documentation skills and general comfort leveraging various technologies to support day to day work
  • Ability to juggle many tasks and switch between visiting with residents and coordinating behind the scenes
