Location: Santa Rosa, California 95401

Job Type: Full-time, W-2

Work Setting: Residential detox treatment center

Schedule: Tuesday-Saturday mixed schedule

Pay: $27-$30/hour, depending on experience and credential


Overview

Avento Health is recruiting a full-time Case Manager for a residential detox treatment setting in the Glen Ellen, California area. This role supports clients throughout the treatment process through assessment, care coordination, treatment planning, documentation, admissions support, and collaboration with the clinical team.

This is a strong opportunity for a behavioral health professional with substance use treatment experience who is passionate about helping clients and families navigate recovery in a structured residential environment.


Responsibilities

  • Assess clients before and during treatment to support appropriate care planning.
  • Serve as the primary case manager for assigned clients and help guide progress through treatment milestones.
  • Develop, update, and document individualized treatment plans and client progress notes.
  • Support admissions, program orientation, and communication with families and referral sources.
  • Assist with medical, legal, and referral coordination as needed.
  • Observe client self-administration of medications and document responses accurately.
  • Maintain timely, accurate, and compliant documentation in client records.
  • Collaborate with interdisciplinary team members and communicate important client updates across shifts.
  • Participate in aftercare support activities as needed.


Required Qualifications

  • High school diploma or GED plus specialized training in chemical dependency, or higher level education.
  • Experience working with clients in substance use treatment, chemical dependency counseling, or related behavioral health settings.
  • Registered or trainee credential such as RADT, SUDRC, RAC, or CATC Trainee, or certified counselor credential such as CADC, SUDCC, or CATC.
  • Strong case management, treatment planning, and documentation skills.
  • Ability to provide culturally responsive care to diverse client populations.
  • Strong written and verbal communication skills.
  • Current First Aid and CPR certification.
  • Ability to work within professional and ethical guidelines.


Preferred Qualifications

  • College-level preparation in counseling, behavioral health, social services, or a related field.
  • Experience in residential treatment or detox settings.
  • Experience working with clients who have co-occurring disorders.
  • Knowledge of 12 Step recovery programs and other peer support models.
  • Licensure as an MFT or LCSW.
