Position Summary

The Case Manager provides critical support by coordinating care, developing individualized service plans, and connecting clients with the resources necessary to achieve treatment goals. Working closely with clinical staff, community providers, and family members, the Case Manager helps ensure continuity of care and supports clients in maintaining long-term recovery and stability.

This role also includes facilitating therapeutic groups and providing individual (1:1) support sessions to reinforce treatment goals and promote client engagement in the recovery process.


Essential Duties and Responsibilities

  • Coordinate care for clients enrolled in IOP and PHP services
  • Conduct biopsychosocial assessments and assist with treatment planning
  • Provide ongoing case management, referrals, and resource coordination
  • Facilitate therapeutic group sessions
  • Provide individual (1:1) support sessions with clients as needed
  • Collaborate closely with therapists, psychiatrists, nursing staff, and external providers
  • Support discharge planning and continuity of care
  • Maintain accurate and timely clinical documentation in accordance with program requirements and regulatory standards
  • Advocate for clients and promote engagement in treatment services
  • Monitor client progress and assist in identifying barriers to treatment success
  • Participate in multidisciplinary treatment team meetings
  • Maintain professional and ethical standards in accordance with company policies and applicable regulations


Qualifications

  • Bachelor’s degree in Social Work, Psychology, Counseling, Human Services, or related field required
  • Master’s degree preferred
  • RAD-T, CADC, or SUDCC certification required
  • Minimum of one year of case management experience in behavioral health, mental health, or substance use disorder treatment preferred
  • Familiarity with Intensive Outpatient (IOP) and Partial Hospitalization Program (PHP) levels of care
  • Strong organizational, communication, and time-management skills
  • Ability to work effectively within a multidisciplinary treatment team
  • Experience with electronic health records (EHR) preferred
  • Knowledge of community resources and care coordination practices
