Job description

We are seeking a hardworking, passionate and caring individual to work with clients that have been diagnosed with a mental illness. As an Adult Case Manager you will help clients gain access to services needed and support them through their recovery.

Responsibilities

  • Maintain a caseload of clients.
  • Coordinate and make referrals for clientย services that will help the individual meet their goals.
  • Complete and accurately update all documentation as required.
  • Work collaboratively with all members of the treatment plan as often as needed.
  • Follow-up on referrals and monitor the clients progress.
  • Provides crisis assessment and intervention services to clients as needed.

Requirements & Qualifications

  • Bachelorโ€™s degree in Social Work, Psychology or a related human services field.
  • BSW applicants are required to be licensed (LSW) prior to beginning employment.
  • Minimum of one yearย providing services to individuals with a mental illness or substance use disorder required.
  • Must qualify as a Mental Health Practitioner in the State of MN
  • Experience working with severe emotional disturbancesย inย adults is preferred.
  • Valid driver's license and proof of auto insurance.
