• Collaborate with a multidisciplinary healthcare team, including physicians, nurses, and chaplains, to devise and implement comprehensive care plans that address the medical, emotional, and psychological needs of patients.
• Conduct thorough psychosocial assessments to identify client needs, develop individualized treatment plans, and provide evidence-based interventions.
• Educate clients and their families about available mental health resources, coping strategies, and understanding the emotional implications of terminal illness.
• Engage in regular case consultations to adapt treatment approaches based on patient progress and changing circumstances.
• Maintain accurate and timely documentation of assessments, treatment plans, and client interactions in accordance with regulatory standards and organizational policies.
• Stay current with evolving practices in mental health care, actively participating in continuing education and professional development opportunities.

In this role, you will primarily work with terminally ill patients and their families, addressing complex emotional and psychological challenges associated with end-of-life care. You will support individuals from diverse backgrounds, tailoring your interventions to meet a wide range of cultural and personal needs, ensuring that each patient receives compassionate and individualized care.

• A Master’s degree in Social Work from an accredited institution.
• Current and valid LCSW license in the state of Virginia.
• At least 2 years of clinical experience, particularly in geriatric care or hospice settings.
• Outstanding interpersonal skills with a proven ability to work effectively within a team setting while demonstrating resilience under pressure.
• Proficient in utilizing electronic health record (EHR) systems for documentation and patient management.

• Prior experience delivering grief support or psycho-social support in a hospice, palliative care, or similar environment.
• Familiarity with community resources, mental health services, and support programs available for patients and families facing terminal illness.

The position offers a competitive salary ranging from USD 80,000 to 90,000 annually, commensurate with experience. In addition to a generous salary, we offer an attractive benefits package that includes:
