SKILLED NURSING & OUTPATIENT THERAPY!
As an OT in this role, you will become an integral teammate in our continuum of care, where we offer SNF therapy as well as 1-on-1 outpatient therapy to patients from outside of the SNF!
Why choose the hybrid role of SNF/OP?
This position is great for any therapist - from a new grad who wants to get exposure to a variety of settings, to a seasoned therapist who is excited about providing a wide range of treatments. The SNF/OP role allows you to flex your treatment muscles and see a wide range of diagnoses and treatment approaches! In the SNF, you can expect to provide 1-on-1 treatments, concurrent therapy, and group therapy as appropriate; and on the OP side you can expect to provide individualized 1-on-1 sessions for 45-60 minutes either with patients who recently discharged from the facility, or patients from the community! This role will flex with SNF time as caseloads fluctuate, meaning you don't have to worry about inconsistent hours!

Facility Location: Sterling Heights - 38200 Schoenherr Road Sterling Heights, MI 48312
This is a hybrid role providing outpatient therapy services in conjunction with SNF based therapy services.
Outpatient and SNF services are paid at an hourly rate.
General Summary:
In this role you will provide occupational therapy services to patients impaired by physical and/or cognitive dysfunction, including assessment, treatment planning and implementation of treatment plans. Participates in patient care conferences, program development, and screenings as appropriate. Provides supervision for OTAs and aides as assigned. Completes appropriate documentation and adheres to policies, procedures and current standards of clinical practice.
Your Responsibilities:
  • Conducts occupational therapy screenings and evaluations to determine patient independence with activities of daily living, including self-care skills, work and leisure.
  • Provides occupational therapy treatments consistent with the professional and ethical standards of the AOTA for an Occupational Therapist.
  • Develops and implements treatment plans using adaptations, strengthening and retraining to reach patients' highest potential for independence. Adheres to professional standards of practice.
  • Provides direct patient care in accordance with productivity standards by use of graded activities to facilitate the restoration of physical and functional abilities.
  • Checks, instructs, assigns and reviews the work of OTAs and aides as assigned.
  • Provides documentation to maintain quality of the medical record as it relates to accuracy, continuity of care and reimbursement guidelines.
  • Participates, as requested, in patient care conferences, per company guidelines.
  • Participates in CQI activities as required ensuring continuous quality of care.
  • Promotes current standards of clinical practice through participation in continuing education and in-service programs.
  • Coordinates and/or participates in in-service programs for internal/external groups.
  • Participates in new program development.
  • Participates in additional activities directed to facilitate effective departmental operations.
  • Participates in company meetings as directed.

Required Qualifications:
  • Successful completion of an accredited Occupational Therapy program
  • Licensure as required by the State of Michigan

Skills and Abilities:
  • Initiative and flexibility in work environment
  • Ability to exercise professional judgement and initiative when analyzing problems and recommending solutions.
  • Ability to react effectively to the needs of patients and their families.
  • Physical Ability to bend, position, transfer and lift patients weighing from 80 pounds to over 300 pounds, without restriction, utilizing lifting devices clinically recommended for the patient.
