Job description

FLSA STATUS: Exempt, Part-Time or Full-Time

REPORTS TO: Associate Medical Director for Family Medicine

DEPARTMENT: Family Medicine

WORKSITE: Ravenswood Family Health Network, All Sites

WORK AUTHORIZATION: Applicants must be legally authorized to work in the United States. Please note that we are not able to provide Visa Sponsorship for this position now and in the future.

POSITION SUMMARY

The Physician implements the mission, philosophy and goals of Ravenswood Family Health Network (RFHN) across all activities. Delivers outpatient primary care commensurate with medical degree and privileging, using evidence based high quality care and a Patient Centered Medical Home (PCMH) framework. Works with other providers to define best clinical practices and with the care team to deliver care under established protocols. Provides comprehensive care that includes wellness, health maintenance, and primary prevention as well as management of chronic diseases and acute care. Works with team and health center to document so quality data is entered and reports are reviewed to improve care, and for reporting to various insurances and agencies, and achieving and maintaining PCMH standards. Maintains a professional atmosphere that respects patients and staff. Champions customer service and quality improvement. Provides consultation and clinical support to Nurse Practitioners, Physician Associates, and other Physicians as needed.

Include board certification; excellent communication skills; ability to work effectively with culturally diverse clinic staff and patients; familiarity with electronic health records (RFHN uses Epic). Bilingual English/Spanish proficiency is highly desirable.
