Job Title: Quality Assurance Assistant – Supportive Housing

Department: Supportive Housing Residential Services
About the Role:

Favarh is seeking a detail-oriented and collaborative Quality Assurance Assistant to join our Supportive Housing team. In this role, you’ll help maintain and improve systems that ensure compliance with state regulations and contractual requirements. You’ll audit documentation, assist with program reviews, and support staff training and development. This position works closely with other departments—including Facilities, IT, and Fiscal—to ensure that programs run smoothly and meet the highest standards of care and accountability.

Key Responsibilities:

Quality Assurance & Compliance:

  • Audit Therap documentation (Individual Support Plans and billing) daily and report findings to Managers and Residential Leadership.

  • Assists in completing and monitors timely submission of documentation, billing, and scheduling related to Individual Plans (IPs).

  • Review and audit staff training compliance monthly; communicate upcoming or expired training requirements to staff and leadership.

  • Upload and maintain staff training records in Relias.

  • Ensure Therap systems are up to date for all individual intakes.

  • Assist in maintaining compliance with Therap and Sandata documentation.

  • Track progress reports and ensure completion and submission to DDS prior to IP meetings.

Health & Safety Monitoring:

  • Conduct bi-weekly safety walkthroughs in Supportive Housing programs and vehicles.

  • Coordinate vehicle maintenance and safety needs as they arise.

  • Collaborate with managers and nursing staff to review monthly health and appointment checklists.

  • Ensure compliance with DDS Minimum Preventive Care Guidelines and HIPAA standards.

Systems & Coordination:

  • Recommend and support implementation of system improvements with Manager and Director approval.

  • Act as a liaison between Residential Services and other departments (IT, Facilities, Fiscal) to support projects and workflow consistency.

  • Undertake additional duties as assigned by management.


Minimum Qualifications:
  • High School Diploma or GED required; Associate degree preferred

  • At least 1 year of experience in Health, Education, Rehabilitation, or Residential Services

  • Proficient in Microsoft Office, particularly Excel

  • Experience with Sandata preferred

  • Strong organizational skills and attention to detail

  • Excellent written and verbal communication skills

  • Ability to work collaboratively across departments

  • Valid Connecticut driver’s license with a clean driving record


Physical & Work Environment Requirements:
  • Ability to stand, walk, and move frequently during site visits

  • Must be able to operate agency vehicles

  • Occasional lifting or moving of materials may be required-must be able to lift up to 75 lbs. 

  • Work takes place in program sites, community settings, and office environments
