Job description

Position Summary

Healthcare Agency Corp is seeking compassionate, dependable, and professional Personal Care Providers/Homemakers to provide high-quality in-home care services to clients in accordance with their individualized Plan of Care (POC). Caregivers assist clients with activities of daily living (ADLs), instrumental activities of daily living (IADLs), companionship, and maintaining a safe, clean, and comfortable home environment while promoting independence, dignity, and quality of life.


Essential Duties and Responsibilities

Personal Care Services (ADLs)

  • Assist with bathing, showering, sponge baths, and personal hygiene.
  • Assist with dressing and undressing.
  • Provide grooming assistance, including hair care, shaving, and oral care.
  • Assist with toileting, incontinence care, and changing briefs.
  • Assist with mobility, ambulation, transfers, and positioning.
  • Assist with range-of-motion exercises as directed in the Plan of Care.
  • Assist with eating and feeding when authorized.
  • Provide medication reminders (does not administer medications unless authorized by state regulations and agency policy).
  • Monitor and report changes in the client’s physical, mental, or emotional condition.
  • Maintain client safety and implement fall prevention measures.
  • Provide companionship and emotional support.


Homemaker Services (IADLs)

  • Meal planning and preparation according to dietary needs.
  • Grocery shopping and essential errands (when authorized).
  • Light housekeeping, including:
    • Sweeping and mopping floors
    • Vacuuming
    • Dusting
    • Cleaning kitchens and bathrooms
    • Taking out trash
    • Changing bed linens
    • Making beds
  • Laundry, washing, drying, folding, and putting away clothing and linens.
  • Organizing household items and maintaining a clean, safe environment.
  • Assist with pet care if included in the client’s Plan of Care.
  • Accompany clients to appointments, community activities, or errands when authorized.


Documentation

  • Accurately complete daily visit notes and electronic visit verification (EVV) requirements.
  • Document services provided according to agency policy.
  • Report incidents, injuries, safety concerns, and changes in client condition immediately.
  • Maintain confidentiality in accordance with HIPAA regulations.


Qualifications

  • High school diploma or equivalent preferred.
  • Previous caregiving or home care experience preferred but not required.
  • CPR/First Aid certification (or willingness to obtain).
  • Valid driver’s license, reliable transportation, and proof of insurance (if transporting clients).
  • Ability to successfully complete agency orientation, competency evaluations, and required training.
  • Ability to pass all required background checks.

  • Ability to stand, walk, bend, kneel, crouch, and reach for extended periods.
  • Ability to assist clients with transfers and mobility as outlined in the Plan of Care.


Work Environment

Services are provided in clients’ homes throughout the community. Caregivers may be exposed to varying household environments and must adhere to all agency safety and infection control policies.
