Physical Therapy Aide - Full Time

Montebello Care Center, a premier skilled nursing and rehabilitation facility in Montebello CA is seeking a full-time Physical Therapist Assistant to join our rehabilitation team. The PTA will work under the direction of the Physical Therapist to help residents improve strength, mobility, balance, and overall function.

Key Responsibilities:
  • Provide physical therapy treatments as directed by the Physical Therapist.
  • Assist residents with therapeutic exercises, mobility training, transfers, and gait training.
  • Support residents in reaching their therapy goals and improving independence.
  • Complete detailed, accurate, and timely documentation for all treatment sessions.
  • Use PCC and Net Health to document care, track progress, and support effective communication.
  • Communicate resident progress, changes, and concerns to the therapy team.
  • Maintain a safe, positive, and encouraging treatment environment.
  • Work closely with nursing, therapy, and interdisciplinary team members.
Qualifications:
  • Current California Physical Therapist Assistant license.
  • Experience in a skilled nursing or rehab setting preferred.
  • Familiarity with PCC and Net Health preferred.
  • Strong documentation, communication, and teamwork skills.
  • Compassionate, dependable, and resident-focused.
