What is a PSH case manager?

A PSH (Permanent Supportive Housing) Case Manager is a professional who provides support services to individuals and families experiencing homelessness or at risk of homelessness. Their primary role is to help clients maintain stable housing by connecting them to resources such as healthcare, employment, mental health services, and life skills training. PSH Case Managers work closely with clients to develop individualized service plans, advocate on their behalf, and monitor their progress toward self-sufficiency. They often collaborate with landlords, social service agencies, and healthcare providers to ensure clients' needs are met. Effective case management is crucial to helping clients remain housed and improve their overall quality of life.

What skills and qualifications are needed to thrive as a PSH case manager?

To thrive as a PSH Case Manager, you typically need a background in social work or human services, with a relevant degree and experience working with vulnerable populations. Familiarity with case management software, housing databases, and knowledge of community resources is important, and certifications like CADC or LCSW can be advantageous. Strong interpersonal skills, cultural competence, and problem-solving abilities are essential for building trust and effectively supporting clients. These skills and qualifications enable case managers to provide comprehensive, client-centered support that promotes housing stability and improved quality of life.

Job description

Jaydot is hiring! We have an immediate need for a PSH Case Manager Supervisor (Tenant-based) to lead and support the tenant-based case management team in helping people who are transitioning from homelessness thrive in permanent housing, and we'd love for you to join us!
WHO WE ARE
Jaydot is a mission-based social enterprise in DC focused on creating quality housing solutions that address the needs of those impacted by social inequity, particularly those who have experienced homelessness. We believe that housing is a human right, homelessness is a solvable problem, and we can create an equitable, human-centered system where housing solutions are readily adapted to fit the needs of those we serve. For more information, please visit jaydot.org.
POSITION SUMMARY
The PSH Case Manager Supervisor oversees tenant-based case management services to support people who have experienced homelessness in securing and maintaining permanent housing. This work is completed within Jaydot's Permanent Supportive Housing (PSH) Program, funded by a contract with the DC Department of Human Services (DHS). The PSH Case Manager Supervisor directly manages tenant-based case management staff that Jaydot employs under the PSH Program, all with the goal of ensuring the housing stability and well-being of clients exiting homelessness.

  • Manage and provide clinical oversight for up to seven (7) tenant-based PSH Case Managers
  • Monitor and track case management staff engagement with participants
  • Review case notes through the lens of quality of information captured and accuracy
  • Review, evaluate, and approve participant case plans for quality and effectiveness
  • Meet individually with Case Managers to plan and review cases, discuss engagement strategies and evaluate the effectiveness of the Case Management staff and services
  • Provide guidance for case managers on addressing particularly challenging participant situations and assist with connections to more intensive support services, as needed
  • Review and approve all notices issued to participants for accuracy and completion
  • Analyze and ensure Housing Stability Plans are updated quarterly and track participant's progress toward achieving their goals
  • Ensure that quarterly re-certifications are completed and designated software and files are up to date with this information

  • Coaching and modeling: The PSH Case Manager Supervisor makes it their mission to develop others. They delegate skillfully and take the time to offer useful and actionable feedback. Because they lead by example (with our values as their north star), they inspire and motivate others to take action and meet goals. They hold the team accountable in ways that grow the people and our impact, and they make sure people feel appreciated.
  • Person-centered: The PSH Case Manager Supervisor meets participants where they are and fosters a relationship in which they are "walking with" the participant rather than "doing for" them. They are empathetic, even and especially when clients are difficult customers.
  • Adaptability and problem solving: The Case Management Supervisor is always ready to take advantage of unexpected opportunities and address obstacles. They look for the root of both simple and complex problems, so they can seek solutions. They approach work with a spirit of "yes" and adapt as things change, which they often do.
  • Attention to detail: The PSH Case Manager Supervisor aims to leave things better than they found them. As the lead on case management documentation they notice and fix errors that others might overlook. When mistakes happen (which they will!), they address them quickly and look for ways to prevent similar errors.
