Certified Registered Nurse Practitioner / Physician Assistant


Reports to Chief Medical Officer


We expect all of our licensed providers to host learners from time to time, since we are a teaching institution. This is our pipeline for recruiting excellent providers who want to serve the populations we take care of. Every student we accept applies for the chance to be here through a competitive process and is reviewed and assessed for their likelihood of working in an underserved, underresourced area in the future. We want to ensure they fit our mission.  We try to give our newer providers time to get settled before adding them to our pool of available preceptors, and we offer best practices for integrating learners into your clinic without compromising efficiency.


**This position is located in Wilcox County**

Background: Cahaba Medical Care Foundation is a community health center providing medical, pharmacy, dental, and behavioral health services to diverse underserved communities in Jefferson, Bibb, Perry, Chilton, Wilcox, Marengo, and Shelby counties. We are a Level 3 Patient-Centered Medical Home and Joint Commission accredited organization, committed to increasing integration and coordination of behavioral health and primary care. This is an exciting, fast paced practice with a strong mission and commitment to providing high quality care. 


Duties and Responsibilities:

  • Provide primary care to patients under supervision of a physician and following established medical protocols

  • Collaborates with physician preceptor to formulate and review medical protocols

  • Assists with development of operational policies and procedures

  • Assists medical director in supervising other clinic staff

  • Obtains complete or interval history of the patient, identifies patient risk factors, identifies health hazards in the patient's environment and provides health education

  • Performs appropriate physical exams as indicated, obtains appropriate laboratory specimens and analyzes clinical data for appropriate diagnosis and treatment

  • Assures treatment and/or plan of care are consistent with established protocols and treatment is revised as necessary according to patient outcome.

  • Documents all pertinent information accurately, legibly, and in a timely manner.

  • Assures appropriate consent forms are signed and dated. 

  • Refers patients to appropriate services which exceed services of the clinic or scope of practice of the practitioner. Assists in assuring that adequate patient health records are maintained and transferred as required when patients are referred.

  • Seeks out community resources to meet or assist with the patient's need

  • Identifies and considers the cultural patterns, economic status and patient resources in plan of care. 

  • Participates in the development, execution and periodic review of the written policies governing the services the clinic furnishes and provides services in accordance with those policies. Participates in studies, audits, and surveys as requested.

  • May direct and coordinate the Centering Group Visit Programs (CenteringPregnancy, CenteringParenting, CenteringDiabetes)

  • Participates in quality improvement activities as assigned by the QIO or CMO

  • Assists in daily lab, image, and record review for all providers

  • Assists in managing registries including nursing home, coumadin, home visit, Pineview, obstetric, and others as assigned


Qualifications:

  • Certification as a Family Registered Nurse Practitioner or Physician Assistant, with a minimum of two years experience preferred, and preferably in a primary care clinic setting

  • Maintains the requirements to keep a valid state license

  • Submits to the necessary components to maintain privileging within the clinic including background checks, random drug screens, peer reviews by CMO, certification of fitness to perform duties, and willingness to comply with the various quality improvement programs including PQRS, MU, METRIC, etc


Travel

  • From time to time, travel to headquarters in Bibb County may be required.
