Job Summary
The Director of the Critical Time Transition Program for Children, Youth and Families (CTTP) provides leadership and direction to the CTTP team which is responsible for facilitating the safe transition of children/youth from a hospital setting to the community. The Director of CTTP will use a model that is trauma-informed and utilizes behavioral support principles, prioritizes family support and skill-building, and is built on a strengths-based, relationship-focused culture.
Essential Job Functions
- Lead the design, development and implementation of the CTTP program, in alignment with state regulations, program guidelines and agency policy and practice, and in collaboration with hospital partners, external stakeholders and Hillside leadership. Collaborate with CTTP programs throughout the state to give input to OMH on the ongoing evolution of the CTTP program.
- Oversee and direct all program components of the CTTP program, which includes the Critical Time Intervention (CTI) and the Transitional Residential Setting (TRS) teams.
- Demonstrate ability to understand various funding streams and manage complex program budget, including successfully coordinating funding through various payers. Maintain Full Time Equivalents (FTEs) within budgeted parameters and manage program expenses to budget. Participate annually in the budget development process.
- Collaborate with the Local Governmental Unit (LGU), Director of Community Service (DCS)/Mental Health Commissioner and work in collaboration with NYS OMH and other state agencies to provide training and support around the development and implementation of the program.Â
- Build a team of high performing staff with a goal of hiring staff from the most prevalent cultural group of service users and staff with lived experience with the mental health system.Â
- Ensure coordination of child/youth discharge to the community, warm hand-off to community care management and facilitation of the use of the TRS as required for brief periods of stabilization.
- Ensure the team provides both in-reach to the hospital as well as continued connection to the child/youth and family/caregiver after discharge (if applicable) from the Transitional Residential Setting.
- Build and foster professional relationships in order to successfully navigate the service delivery systems within the local communities, including local hospitals and community providers.Â
- Facilitate and maintain a program culture of Diversity, Equity, Inclusion and Belonging. Ensure staff and youth safety, comfort and well-being.Â
- Monitor and respond to child/youth movement, including referral, admissions and discharge. Consult and coordinate with hospital and agency team members to ensure continuity and quality of care.
- Ensure the team works closely with families/caregivers and coordinates with the hospital partner. Foster an intensive care management and family/caregiver engagement approach.Â
- Provide clinical direction and oversight to all clinical aspects of the program. Â
- Participate in OMH-directed training and technical assistance, including but not limited to regular calls, learning collaboratives, etc.
- Utilize all available data, such as PSYCKES, to make sound clinical decisions and to assist with an informed discharge planning approach.Â
- Ensure continuous quality improvement of services and development of the program, including regular monitoring and reporting of program outcomes.Â
- Ensure the care provided to the child/youth in care is compliant with all federal, state, and local statutes, regulatory agency standards and Hillside policies including but not limited to NYS Office of Mental Health (OMH) and Council on Accreditation (COA). Â
- Develop and deliver coordinated services in accordance with general accepted community standards of professional practice as part of an interdisciplinary team.
- Actively participate in agency committees, projects and initiatives, as assigned.
- Make sound critical decisions and have authority to implement decisions in conjunction with the leadership structure of the given area of responsibility.
- Maintain and adhere to ethics of confidentiality. Discuss confidential matters in an appropriate manner in compliance with the Health Insurance Portability Accountability Act (HIPAA).
- Document in each child/youth's electronic clinical record all diagnostic assessments, individual plans of treatment, and treatment notes regarding recommended services and appropriate treatment.
- Assume on call responsibilities on a rotating basis as per facility guidelines, as assigned.

- Governance: Ensures Hillside policies, procedures and all applicable governmental laws/regulations/guidelines are followed. Makes decisions and has authority to implement decisions in conjunction with the leadership structure of the given area of responsibility.Â
Education & Experience
- Master's degree required
- Minimum 5 years of relevant clinical experience post Master's degree required
- Minimum 5 years of leadership or management experience required
- Experience with care coordination and/or hospital and community behavioral health system navigation or other relevant experience required
SPECIAL REQUIREMENTS
- Active, unrestricted Mental Health Practitioner license in NYS required for one of the following:Â
- Licensed Master Social Worker (LMSW), Licensed Clinical Social Worker (LCSW), Licensed Marriage and Family Therapist (LMFT), Licensed Psychologist, Licensed Mental Health Counselor (LMHC), or Licensed Creative Arts Therapist (LCAT)Â
- Unrestricted, valid NYS driver's license for minimum of 1 year with a clean driving record and minimum insurance coverage that meets agency standards.Â
- CPR training within 60 days of hire and recertification annually.
- Ability to travel locally and regionally.Â
- Ability to obtain and maintain Transport for Evaluation (9.58) certification, as applicable.
