The Direct Support Professional - Choice Services is responsible to provide services to individuals with disabilities.

ESSENTIAL FUNCTIONS

The essential functions of this position consist of the ability to write, read, and comprehend what is read.  Advanced expressive and receptive language skills are also required in the execution of the responsibilities below.   Technology skills required.  Must possess a valid driver’s license. Be able to transport individuals and be physically present at each service location.  In fulfilling this job, one must be able to meet the physical requirements.  All the above may be performed with or without reasonable accommodations.

General Work Principles

1. Be a primary advocate for individuals served by ensuring health, safety, and welfare at all times.
2. Honor and respect individuals and their personal rights in all aspects of the individuals’ lives.  Comply with Confidentiality Policy and all applicable HIPAA Regulations.
3. Address, treat, and interact with individuals in a dignified manner at all times.
4. Act as an appropriate role model and present an acceptable appearance and image when representing the agency.
5. Exercise common sense and good judgment in all decision making and contact supervisory staff for direction when necessary and required.
6. Have full knowledge of Integration, Everyday Lives, Hope Enterprises, Inc. Policy and Procedures and current ODP practices and philosophies. 
7. Identify needs and weaknesses of service delivery.  Participate in the development of policies and procedures to improve operation of service delivery. 
8. Promote a trusting relationship and remain professional when working with individuals/families.
9. Facilitate and maintain relationships between the Administrative Entity, Supports Coordination Organization, day programs, and other support services.
10. Avoid allowing personal beliefs and values to influence dealings with individuals receiving services.

Responsibilities

1. Participate in orientation and ongoing training.
2. Ensure compliance with Integration, Everyday Lives, Hope Enterprises, Inc. Policy and Procedures, and current ODP practices and philosophies.
3. Complete the Association of Community Rehabilitation Educators (ACRE) and Community Participation Support training within established timelines as required.
4. Participate in the annual evaluation process.
5. Submit time records, expense vouchers, and other required personnel forms within established time frames.
6. Adhere to established work schedules.  Perform temporary work assignments to alleviate staff shortages and assure consistency in providing services.
7. Meet with supervisor when necessary.
8. Notify and consult with supervisor when additional support is needed.
9. Enhance community/employment connections for individuals by seeking opportunities through regular contacts within local community. 
10. Ensure accurate and timely submission of service documentation.
11. Be familiar with 2380 Regulations.
12. Have full knowledge and comply with Electronic Visit Verification (EVV).
13. Be familiar with Employment Services and Office of Vocational Rehabilitation (OVR) policy and procedures. 
14. Assist with maintaining agency vehicles as required.

 Service Delivery

1. Utilize technology to enhance service delivery.
2. Participate in the referral process.
3. Participate at Individual Support Plan (ISP) Team Meeting. 
4. Be familiar with ISP to include outcomes, health promotions, behavioral support plans, SEEN, etc. Assist the individual to achieve their goals.   
5. Ensure delivery of service to maximize the individual’s budgeted hours/units.                                                                                                           
6. Facilitate, assist, or represent the individual when there are barriers with service delivery.

Notification of Incidents

            A.        Have full knowledge of and comply with Incident Management Policy.

   Other Duties

            A.        Assist with presentations to other agencies, organizations, and interested parties.

            B.        Participate in meetings/committees as assigned.

            C.        Attend and participate in required trainings, seminars, and/or conferences.

            D.        Perform other duties as may be required or assigned.

HOURS: Non-exempt position; up to 40 hours weekly, evenings and weekends may be required.

POSITION SUPERVISED: N/A

High school diploma or General Educational Development (GED) certificate. 

Experience in human services is preferred, or an equivalent combination of experience and training is acceptable.  All staff must be at least 18-years of age and have a valid driver’s license.
