Job description

Key Responsibilities:
  • Manage daily operations and staff in Care Management Department, including care coordination, case management, discharge planning, utilization management, social work services, and pre-admission case management.
  • ct as liaison between patients, families, department staff, physicians, healthcare team, community agencies, managed care representatives, and skilled nursing facilities.
  • Establish, implement, and monitor best practices and clinical standards to ensure quality patient care and organizational financial integrity.
Required Qualifications:
  • Master's Degree in Social Work from an accredited school.
  • Washington Clinical Independent Social Worker License.
  • 6 years experience in Case Management, including acute care setting.
  • 2 years Managerial/Director level experience, preferably in Case Management in a large hospital.
