About This Opportunity

Advanced Behavioral Health (ABH) is seeking a compassionate, mission-driven Community Case Manager to join our DMHAS-sponsored Military Support Program (MSP). This is a meaningful role for someone who is passionate about serving Connecticut's military community, including female veterans, active-duty service members, and military families navigating mental health, substance use, trauma, or life transition challenges.

Working alongside clinicians, military support networks, and community partners statewide (CT), you will provide outreach, care coordination, and resource navigation that makes a direct difference in people's lives.

What You'll Do

• Conduct outreach to connect service members, veterans, and families to behavioral health, housing, healthcare,
and other essential community resources

• Coordinate care across VA systems, DMHAS programs, and community-based providers

• Provide culturally responsive, trauma-informed support with a special focus on female service members and military
families

• Participate in outreach events, community presentations, and military-affiliated partnership activities

• Maintain accurate documentation and support program reporting requirements

• Collaborate with MSP clinical staff and community partners across Connecticut

• Performs other tasks/responsibilities as required to support the business operations

Requirements

What We're Looking For

Required:

• Experience in behavioral health case management or care coordination

• Strong understanding of military culture, veteran populations, and/or VA systems

• Knowledge of trauma-informed care principles

• Excellent interpersonal, organizational, and documentation skills

• Valid CT driver's license and reliable transportation (community-based travel required)

Preferred:

• Bachelor's or Master's degree in Social Work, Counseling, Psychology, or related field

• Personal military service or lived experience supporting military families

• Familiarity with DMHAS programs and Connecticut community resources

• Bilingual (English/Spanish) a plus
