Job description

POSITON SUMMARYย 

As the Program Manager you will serve a dual role in the leadership team.ย  The first role is agency-wide case management for the individuals we serve in your assigned locations.ย  The secondary role is that of providing continuous support, direction, mentoring and training to Site Supervisors.ย  Working with Site Supervisors to plan and implement the most appropriate services for the individuals we serve is a key component of this role.

ESSENTIAL DUTIES AND RESPONSIBILITIES

Case Management Role

  • Serve as the point of contact for all other service providers.
  • Collaborate with the Individuals we serve, Case Managers, MCOs, Guardians, natural supports, and other service providers.
  • Oversee and implement the individualโ€™s plan of care.
  • Develop the agency Individual Service Plan (ISP) that supports the goals and desires of the individuals we serve.
  • Conduct annual and quarterly reviews to re-evaluate the supports provided while advocating for adjustments needed to the Person-Centered Service Plan (PCSP).
  • Collaborate with the individual and their care team to review and evaluate any current rights restrictions in place or in need of implementation.
  • Conduct individual file audits to ensure all information needed is included in the individualโ€™s file.
  • Work with other Program Managers and the Director to evaluate program success, identify barriers to success and promote solutions to improve the services we provide to the individuals we serve.

Leadership Role

  • Oversee operations for all assigned sites.
  • Serve as a coach and mentor to Site Supervisors through training, individual consultation and modeling desired outcomes.
  • Provide constructive feedback through regular meetings and staff evaluations.ย 
  • Work with Site Supervisors to address any house concerns, scheduling concerns or other barriers to their success or the success of the individuals we serve.
  • Collaborate with Site Supervisors to plan and coordinate services for the individuals we serve.
  • Monitor medication management for assigned sites.
  • Oversee the Site Supervisor as they set appointments and arrange transportation for the individuals we serve.
  • Ensure that shift documentation is complete and accurate.ย 
  • Actively participate in selection of new personnel and transfers.

QUALIFICATIONS

Must be at least 21 years of age and meet one of the following conditions: Possess a bachelorโ€™s degree in social services or related field of study and 1 year of related experience or an associateโ€™s degree and a minimum of 3 years of directly related experience or a High School Diploma/GED and 5 years of directly related experience.ย  Previous supervisory experience is required.
