I. BASIC FUNCTION:
Responsible for providing care management for patients of FCHC and for clinical supervision of non-licensed care
managers.

III. PRIMARY RESPONSIBILITIES:
1. Function as the Care Manager within the integrated primary care and behavioral health team.
2. In coordination with the BHC and primary care provider, enroll eligible and interested patients into the Circle of
Care project and track patient progress towards grant outcomes.
3. As often as is clinically appropriate, face-to-face visits and telephone calls with Circle of Care patients and other
members of the patient's network over the course of their enrollment in the project (3-12 months).
4. Establish and maintain a patient registry to monitor clinical outcomes as well as key processes of care.
5. Arrange for inpatient, partial hospitalization, intensive outpatient programs as needed. Assist with discharge
coordination.
6. Coordinate information flow between the PCP, BH providers, CHW, and other FCHC staff within the Circle of Care
program.
7. Attend care team meetings and document clinical information/decision-making in the patient registry.
8. Identify barriers to treatment and possible solutions, including coordinating/offering transportation when needed.
9. Establish and maintain cooperative referral relationships with community agencies, organizations, and
professionals. Together with the other care managers, maintain an up-to-date referral listing.
10. Facilitate referrals to other outpatient therapists as indicated.
11. Facilitate referrals to psychiatry as needed.
12. Participate in staff in-service programs related to social services and/or behavioral health.
13. For LCSWs only: Provide clinical supervision for licensure to LMSWs.
IV. PERIODIC DUTIES:
1. Contributes services to community public health activities as appropriate.
2. Participates in Center work team as needed.
3. Participates in Center staff problem solving groups.
4. Performs other duties as assigned.
V. WORKING RELATIONSHIPS:
Inside Center: All Inclusive.
Outside Center: Patients, community organizations and agencies, professional colleagues and organizations,
and institutions of learning.
VI. QUALIFICATIONS:
1. MSW required. LMSW, LCSW preferred.
2. Minimum of three (3) years experience working in the field of social work or behavioral health care is required.
3. Work experience or strong desire to work in urban community health setting preferred.

VII. CONTINUING EDUCATION:
1. Active professional development as demonstrated through attendance of seminars, continuing education
programs, etc. is strongly recommended.
2. Satisfactory maintenance of continuing education standards as established by the Division of Professional
Registration of Missouri is required for LMSW and LCSW.
