Job description

Job Title: Peer Specialist

Department: Behavioral Health

Location: SLH - GH Main (40)

Hours Per Week: 40

Schedule: Monday - Friday; Days, 8:00AM - 4:30PM or 9:00AM - 5:00PM

SUMMARY:

Peer Specialists are fully integrated team members who promote client self -determination and decision making. Provide essential expertise and consultation to the entire team to promote a culture where each client's point of view is recognized, understood, respected and integrated into treatment rehabilitation and community self help activities.

RESPONSIBILITIES:

  • Assist with the client's participation in developing the Individualized Recovery Plan and help the client understand the diagnoses and plan components.
  • Carry out duties as a member of the team, under the direction of the Program Manager and/or Coordinator.
  • Must maintain client confidentiality in accordance with the RGHS policy.
  • Provide support and encouragement to help clients socialize, interact in a milieu setting and facilitate social activities groups within the milieu.
  • Assist consumers who have experienced social stigmatization with their sense of acceptance and affiliation.
  • Provide practical help and mentoring, advocacy, coordination, side-by-side individualized support, problem solving and direct assistance to help clients obtain the necessities of daily living.
  • Promote client self advocacy skills
  • Participate in staff meetings and treatment review meetings with PROS team, MD and NP.
  • Collaborate with the team to document consumer progress to maintain a permanent record of client activities according to the agency's established methods and procedures.

REQUIRED QUALIFICATIONS:

  • New York State Certified Peer Specialist required
  • Valid NYS Drivers License required
  • Reliable transportation required
