Position Summary

We are seeking an experienced Home Health Registered Nurse (RN) to provide skilled nursing care to patients in their homes. The RN will perform comprehensive assessments, manage individualized care plans, complete OASIS documentation, and collaborate with the interdisciplinary team to ensure high-quality, patient-centered care.

Key Responsibilities
  • Conduct comprehensive nursing assessments in the home setting.
  • Perform Start of Care (SOC), Resumption of Care (ROC), Recertification, Follow-Up, and Discharge visits.
  • Develop, implement, and update individualized Plans of Care (POC).
  • Complete accurate and timely OASIS documentation using Homecare Homebase (HCHB).
  • Administer medications, IV therapy, wound care, catheter care, venipuncture, and other skilled nursing interventions.
  • Monitor patient progress and communicate changes in condition to physicians and the care team.
  • Educate patients and caregivers on medications, disease management, pain management, and home safety.
  • Coordinate care with physicians, therapists, social workers, and other healthcare professionals.
  • Supervise Home Health Aides (HHAs) and Licensed Practical Nurses (LPNs/LVNs) as appropriate.
  • Participate in interdisciplinary case conferences and quality improvement initiatives.
  • Ensure compliance with Medicare, CMS, agency policies, and infection control standards.
Required Qualifications
  • Active Registered Nurse (RN) license.
  • Associate Degree in Nursing (ADN) or Nursing Diploma from an accredited program.
  • Minimum 2 years of Home Health experience.
  • Homecare Homebase (HCHB) experience required.
  • OASIS experience required.
  • Current CPR/BLS certification.
  • Valid driver's license, reliable transportation, and current auto insurance.
Preferred Qualifications
  • Bachelor of Science in Nursing (BSN).
  • Certified Hospice and Palliative Nurse (CHPN) certification.
  • Excellent assessment, documentation, communication, and time management skills.
  • Ability to work independently while managing a home health caseload.
