  • Case Management: Develop and maintain Individual Support Plans (ISPs), ensuring timely follow-up and delivery of support services.
  • Team Leadership: Supervise and mentor staff, evaluate performance, and provide contextual training in therapeutic models.
  • Behavioral Support: Design and implement behavior management interventions in collaboration with the clinical team.
  • Program Oversight: Act as the site's contact person in the absence of management, ensuring smooth operations and program integrity.
  • Community Engagement: Collaborate with families, guardians, and external agencies to promote individual progress in home, work, and community settings.

Qualifications
Education & Experience:
  • Bachelor's degree preferred.
  • 1-3 years of related experience, including staff supervision and case management.
  • Valid standard driver's license (Class C or D).
Skills & Knowledge:
  • Expertise in activity planning, behavior management, and therapeutic techniques.
  • Strong understanding of person-centered planning and daily living assistance.
