Job Status: This position is full-time. HRDC also offers flexible schedules and a "reduced work week", which encourages full-time staff to use up to 4 paid hours weekly for their personal wellness.

Wage: $27.00/hour

General Summary: The Blueprint Transitional Housing Coordinator is a vital part of youth programming and is responsible for coordinating the operations of the Blueprint (Youth) Transitional Housing property and delivery of case management to the residents there. Blueprint Transitional Housing is a semi-independent, communal living facility that supports 16-24 year-olds for up to two years. This role involves coordinating the delivery of supportive housing services with various professionals, internal programs, and community partners. The coordinator ensures that program activities meet organizational goals, maintains program expectations, and works closely with Resource Property Management (RPM) for facility operations to support seamless program function.

The position includes coordinating and assessing potential young adult participants experiencing homelessness, monitoring their daily progress, facilitating house meetings, and coordinating case conferences with case management team members. The program works with two properties on one central campus that accommodate different youth populations of various ages and development levels. One of the transitional homes works directly with our minor population and adheres to strict confidentiality and reporting requirements.

Primary Job Duties and Responsibilities:

1. Program Coordination (35%)

• Implement a program philosophy built on providing residents safety, freedom, choice, and accountability

• Foster a positive shared-living environment by helping participants navigate conflict, practice communication and problem-solving skills, and build community with one another.

• Coordinate with the Compliance Department to update policies and procedures to ensure efficient and compliant operations of Blueprint Transitional Housing

• Collaborate with guardians, schools, legal supports, property management, behavioral health providers, and community partners as appropriate

• Coordinate with the Resource Property Management team to ensure facility maintenance and room turnover.

• Coordinate with community partners to provide onsite services, education, and opportunities for community connection based on participants needs and goals

• Facilitate house meetings, life skills activities, conflict resolution, and community engagement opportunities that support residents’ independent living and well-being

• Create opportunities for participants to share house activities, community expectations, and other aspects of program life that directly impact them

• Lead weekly resident house meetings, structuring overnight staff attendance, partner education attendance, and resident wellness activities, all within the weekly meeting

• Support and coach participants in building independent living skills including completing household responsibilities, and program expectations

2. Housing & Tenancy Support (35%)

• Support participants in transitioning from homelessness into transitional housing, including adjustment to the program, development of independent living skills, and establishment of housing stability

• Partner with participants to develop individualized goals and housing stability plans, with increasing focus on permanent housing, education and employment, income, wellness, and supportive connections

• Center participant voice and choice in goal planning, recognizing youth as the experts in their own lives and supporting them in defining and pursuing goals that are meaningful to them

• Provide Tenancy Support Services and HUD housing counseling activities, as applicable, to support youth in identifying, securing, and successfully transitioning to permanent housing

• Connect participants to mainstream benefits, education and employment resources, healthcare, behavioral health, and other community supports to address barriers and advance youth-identified goals

3. Data and Documentation (5%)

• Maintain accurate and timely participant files, case notes, service plans, assessments, releases, and other required program documentation

• Coordinate with Compliance to ensure data and documentation meet reporting, monitoring, and program requirements

• Enter and maintain required data in HMIS and other program databases in accordance with agency and funding requirements

4. Staff Supervision (15%)

• Supervise, coach, and support overnight program staff to ensure consistent and effective program operations and service delivery

• Provide training, performance feedback, and evaluation in accordance with agency practices

5. Training and Development (5%)

• Engages in ongoing training opportunities related to best practices in family case management, housing stability, trauma-informed care, and organizational policies.

• Maintains knowledge of community resources, benefit programs, and funding requirements to ensure high-quality service delivery.

• Participates in required certifications, meetings, and professional development activities.

6. Organizational Participation (5%)

• Attend all organizational team meetings

• Read the weekly news to stay informed

• Participate in trainings and wellness days when available

• Conduct respectful, professional, and effective conversations with customers and volunteers regarding behavioral expectations, program compliance, and HRDC property standards.

• Ability to define problems, collect data, establish facts, and draw valid conclusions

Confidentiality:

This position requires regular handling of sensitive and confidential client and agency information. Unauthorized disclosure could harm client interests and the organization, and may also violate state and federal privacy laws. This position may have access to reports, records, and personnel files, and must exercise the highest level of discretion, judgment, and confidentiality at all times.

Computer Skills:

• Proficient in navigating spreadsheets, multiple databases, email, calendar
• Willingness to learn new software systems
• Comfortable with Microsoft and Google operating systems

Drivers License:

• Valid State Driver License required
• Reliable vehicle and appropriate liability insurance are required

Licenses or Certifications That Would Be Useful but Are Not Required:

• CPR and First Aid, Trauma Informed Care, Mental Health First Aid

• HUD Housing Certified within 6 months of hire

• The incumbent must be able to perform multiple tasks simultaneously, must be able to work under time and/or deadline pressures.

Education and Experience:

• Bachelor’s degree in Psychology/Social Services/Education or related field, from a four-year college or university preferred or comparable life/work experience.

Supervision Received:

• This position operates under general supervision.
• The incumbent’s work is primarily assigned by daily workflow and operations.
• The incumbent is authorized to make decisions as to the timing and sequence of work and the work is performed according to established policies and procedures and/or within applicable laws, rules and regulations governing the tasks being performed.

Supervision of Others:

• This position supervises the Blueprint Housing Specialist & Overnight Relief Aids.

• May be responsible for training, recruiting, managing, and scheduling the work of volunteers.

• Daily interaction with the public or consumers
• Interacts with program managers, executives, HRDC boards and/or committees several times a year
