Job description

The Center for Human Development, (CHD) is seeking an experienced and creative Case Manager at its Two Rivers Recovery Center in Greenfield. Two Rivers is a program for women in early recovery from substance use who are pregnant or postpartum mothers with their infants. The program supports up to 25 women and 5 infants in long term (6 to 12 months) residential treatment. The comprehensive clinical service is a trauma informed, harm-reduction model.

Your role as a Case Manager:

The Case Managers is responsible for the development and implementation of person-centered treatment plans, facilitating evidenced-based therapeutic group, individual therapy, assistance with discharge planning, as well as community support coordination and collaboration. The Case Manager receives supervision from a professional, experienced Clinical Director.

 Requirements:

  • High school diploma or equivalent required
  • Bachelor’s level required
  • Knowledge of substance use disorders and treatment
  • Experience facilitating groups
  • Must pass background screening process including CORI
  • Valid driver’s license and have a clear driving record check
  • Have reliable transportation, for occasional business use

Starting Salary: $24/hr.-$28/hr.
