Position Summary:
We are seeking an experienced and empathetic Nurse Practitioner (NP) to join our team as a Remote Care Manager. In this role, you will engage with patients virtually to assess, coordinate, and monitor their health care needs-playing a vital part in our mission to enhance quality of life through proactive care management.
Key Responsibilities:

  • Conduct remote patient assessments with empathy and professionalism
  • Monitor patients' health status and escalate warning signs appropriately
  • Apply medical terminology knowledge to assess and coordinate care
  • Communicate clearly and compassionately with patients and families
  • Document thoroughly and accurately within multiple computer systems
  • Prioritize daily responsibilities while working independently
  • Provide excellence in patient care and customer service
  • Maintain patient confidentiality and HIPAA compliance at all times
Qualifications:
  • Current, active NP license in good standing
  • Minimum 3 years of experience in a clinical or patient-facing setting
  • Minimum 3 years of experience in care management or case coordination
  • Prior experience working remotely preferred
  • Superior verbal communication and active listening skills
  • High proficiency in medical terminology
  • Excellent organizational and documentation skills
  • Professional phone etiquette and strong interpersonal abilities
  • Ability to recognize clinical red flags and escalate care as needed
  • Tech-savvy; able to work within multiple systems efficiently
  • Must have a HIPAA-compliant, private work area at home
