Job Summary:
The ideal candidate provides comprehensive support and guidance to clients as they navigate the recovery process at Freedom Recovery Centers. This role involves coordinating individualized care plans, connecting clients to essential resources, and supporting them in achieving their recovery goals, extending lengths of stay for positive outcomes. The Case Manager serves as a key advocate for clients, collaborating with clinical and community partners to address client needs in a holistic and client-centered manner.
Key Responsibilities:
  • Client Assessment and Care Planning
  • Conduct initial assessments to determine each client’s needs, strengths, and goals, including risk factors, social support, and resource requirements.
  • Develop and implement individualized care plans in collaboration with the clinical team, ensuring plans align with each client’s recovery goals.
  • Regularly review and update care plans based on client progress and evolving needs.
  • Resource Coordination and Referrals
  • Connect clients with internal and external resources, including housing, employment, medical care, mental health services, and community programs.
  • Coordinate referrals to specialized services, such as vocational training, educational programs, or legal assistance.
  • Build and maintain relationships with community agencies, healthcare providers, and support organizations to facilitate client access to services.
  • Client Advocacy and Support
  • Serve as a primary advocate for clients, helping them overcome barriers to recovery and access necessary resources.
  • Provide counseling, motivation, and encouragement to clients, empowering them to actively participate in their treatment and recovery journey.
  • Assist clients in developing life skills, coping strategies, and support networks to enhance their resilience and self-sufficiency.
  • Progress Monitoring and Documentation

  • Crisis Intervention and Risk Management
  • Identify and respond to potential crises or high-risk situations, following protocols to ensure client and staff safety.

  • Supportive to the company’s vision and approach of longer lengths of stay and positive outcomes for lasting recovery
  • Document and report any incidents or safety concerns according to facility guidelines and regulatory requirements.
  • Client Discharge and Aftercare Planning
  • Coordinate discharge planning to ensure clients have a safe and supportive transition back to the community.
  • Develop aftercare plans, including follow-up appointments, support groups, and community resources to support ongoing recovery.
  • Conduct post-discharge check-ins as necessary to monitor client progress and provide continued support.
Qualifications:
  • Bachelor’s degree in Social Work, Psychology, Counseling, or a related field; Master’s degree preferred.
  • Certified Substance Abuse Counselor (CSAC- A, CSAC-S) certification required.
  • Minimum of 2 years of experience in case management, preferably in a substance abuse or behavioral health setting.
  • Knowledge of substance abuse treatment, recovery resources, and community-based services.
  • Strong understanding of HIPAA, confidentiality standards, and ethical guidelines in client care.
  • Proficiency in electronic health records (EHR) and case documentation.
