Position Summary:
The Home Health Registered Nurse (RN) provides skilled, patient-centered nursing care in the home setting. This role focuses on comprehensive assessment, treatment, patient education, and care coordination to promote patient safety, recovery, and independence. The RN practices autonomously while collaborating closely with physicians, therapists, and the interdisciplinary care team.
Key Responsibilities:
• Conduct comprehensive in-home patient assessments, including physical, emotional, and environmental evaluations
• Develop, implement, and revise individualized Plans of Care based on patient needs and physician orders
• Deliver skilled nursing services in accordance with established standards of practice
• Monitor patient conditions and promptly identify and report changes requiring clinical intervention
• Educate patients and caregivers on disease processes, medications, treatments, and self-care strategies
• Coordinate care with physicians, therapists, and interdisciplinary team members
• Maintain accurate, timely, and compliant clinical documentation
• Manage time effectively across multiple patient visits
• Respond rapidly to changes in patient condition while working independently
Required Qualifications:
• Active, unrestricted Registered Nurse (RN) license in the state of Wisconsin
• Associate's or Bachelor's degree in Nursing (BSN preferred)
• Prior home health or community-based nursing experience preferred
• Strong clinical assessment, critical thinking, and decision-making skills
• Ability to work independently without immediate on-site supervision
• Excellent communication, organization, and time-management skills
• Bilingual Spanish/English preferred
