Responsibilities

  • The Care Coordinator works in collaboration with providers, clinic nurses, practice managers, and other healthcare professionals to develop, implement, evaluate, and revise the delivery of care coordination services to those patients and patient populations determined to be at high risk or in need of specific care/quality-related services.
  • The care coordinator is responsible for collaborating directly with providers, community agencies, and acute-care staff/providers to create consistent and coordinated delivery of high-quality healthcare services to targeted populations.
  • Actions will be directed toward the goals of prevention, assessment, risk reduction, and health status improvement.
  • Demonstrates the ability to connect to and form a relationship with patients and families from a variety of backgrounds, socio-economic conditions, and cultures to assist in and promote optimal health attainment.
  • The Care Coordinator works within the context of a patient-centered, team approach and in continuous partnership with patients, families, and providers.

Qualifications

Job Requirements:

  • Current Kansas LPN or RN license
  • BLS certification must be obtained within 14 days of hire or transfer into the role and prior to providing direct patient care

Preferred Job Requirements:

  • Experience in comparable healthcare setting
