Community Health Navigator
Location: Edmonds WA/Sonomish County area
FLSA Status: Exempt
Job Status: Full Time
Work Model: Remote/Field Based
Work Schedule: Monday-Friday with occasional evenings and/or weekends
Vehicle Required: Yes - with valid insurance
Amount of Travel Required: 25-50% in 30 mi community service area
Reports To: Care Team Manager
At Reema Health, we believe a personalized, community-based approach to care makes it easier for anyone to get what they need to improve their lives. We believe a relationship with a real person, someone who listens, meets members where they are, and understands the community's resources , can ease those burdens.
As a Community Health Navigator, or as we internally refer to as a Community Guide, you'll build genuine relationships with members while actively guiding them towards greater engagement in their own healthcare. This is purposeful work and you'll partner with members to address social barriers so that each member can accomplish health activities that improve their wellbeing. You'll work through Reema's technology platform daily to reach members, schedule visits, and document your work. As a growing start up, roles evolve quickly. We look for Community Guides who embrace fluidity as an opportunity to grow and innovate.
Outreach & Engagement
  • Conduct phone and in-person outreach and enroll members in Reema's care model
  • Build and manage a caseload (80-120 members), providing coverage for other Guides during absences or workload spikes
  • Assess member needs, including social determinants of health, to build a member-centered care plan

Support Members Toward Their Goals
  • Establish trusting relationships and reliably follow through in a timely, appropriate manner
  • Help members understand why a health activity matters, work through hesitation with empathy, and follow through to completion
  • Help members steadily overcome barriers to care, with visible progress every month before the program ends
  • Coordinate referrals and support appointment scheduling and attendance
  • Independently organize and prioritize your workload to hit monthly goals and daily metrics
  • Proactively engage with members using a comprehensive mix of outreach channels, including phone, text, email, and in-person visits
  • Connect members to community and health resources, including transporting or accompanying them as needed
  • Maintain professional boundaries and confidentiality; consult the multidisciplinary team on complex situations
  • Document all member work thoroughly and on time in the Reema App, including keeping care plans current
  • Comply with HIPAA and protected health information confidentiality standards

  • Work independently and collaboratively in a fast-paced startup - see frequent change as an opportunity, not a barrier
  • Serve members with dignity and respect; build and maintain relationships with community resources

Also Valued - Flexible on Experience Level
  • 1+ years of direct experience with the population/community served, or equivalent
  • Bachelor's degree in Social Work, Human Services, Psychology, Health Sciences, or related field (or equivalent direct experience)
  • Experience with crisis planning, case management, and professional boundaries in community-based work
  • Relationship-building, community resource knowledge, and health system navigation
  • Comfort working as part of a multidisciplinary team

  • 18 years of age or older
  • Available to work Monday-Friday during core business hours.
