Uniontown Nursing & Rehabilitation is seeking a compassionate and dedicated Full-Time Occupational Therapist (OT) to join our in-house rehabilitation team in Uniontown, Pennsylvania. This is an excellent opportunity to work in a collaborative Skilled Nursing and Rehabilitation environment where you'll help residents regain independence, improve function, and enhance their quality of life.

Responsibilities
  • Evaluate residents and develop individualized occupational therapy treatment plans.

  • Provide skilled occupational therapy services to short-term rehabilitation and long-term care residents.

  • Address activities of daily living (ADLs), upper extremity function, cognitive skills, adaptive equipment needs, and functional mobility.

  • Monitor resident progress and modify treatment plans as appropriate.

  • Educate residents, families, and caregivers on therapeutic interventions and safety techniques.

  • Complete accurate and timely documentation using Net Health and PointClickCare (PCC).

  • Collaborate with physicians, nursing staff, and the interdisciplinary care team to maximize resident outcomes.

  • Maintain compliance with all federal, state, and facility regulations.

Qualifications
  • Current Pennsylvania Occupational Therapist (OT) license.

  • Graduate of an accredited Occupational Therapy program.

  • Skilled Nursing Facility (SNF) or Long-Term Care experience preferred, but new graduates are encouraged to apply.

  • Experience with Net Health and PointClickCare (PCC) is a plus.

  • Strong communication, clinical reasoning, and organizational skills.
