Role Overview
We’re seeking Primary Care NPs to provide on-site care to Assisted Living/RCFE residents across our partner communities. This role offers steady, predictable patient volume and the opportunity to deliver high-quality care directly where seniors live.

What You’ll Do

  • Routine & urgent assessments; chronic disease management
  • Post-discharge follow-ups, med reconciliation, labs/diagnostics
  • Coordinate with facility staff, specialists, families/POAs
  • Same-day documentation; CMS/regulatory compliance

Schedule & Setting
Full-time position with a structured weekday schedule. You'll spend 3 days delivering on-site care with strategically clustered routes designed to minimize travel and maximize time with patients. The remaining 2 days are in-office at our Irvine headquarters for administrative and documentation tasks.

Qualifications

  • Active CA NP & RN licenses; active furnishing license; NPI, DEA, BLS
  • Geriatrics or facility-based experience preferred; EMR proficiency
