Are you an experienced home health clinician, or are you looking to transition or supplement your income from outpatient, SNF, or hospice settings? We are looking for licensed Physical Therapists (PT), Physical Therapy Assistants (PTA), Occupational Therapists (OT), and Certified Occupational Therapy Assistants (COTA) to join our growing network in Pima County. What You'll Do
  • Provide Skilled Home Health: Deliver individualized physical or occupational therapy services (evaluations, treatments, or assistant-level plans of care) in the patient's home environment.
  • Perform Therapeutic Interventions: Focus on functional strengthening, gait training, ADL retraining, balance, and fall risk reduction to help patients reach their independence goals.
  • Report & Collaborate on Patient Progress: Document patient responses to treatment and maintain seamless communication between therapists and assistants to ensure a coordinated approach.
  • Manage Your Caseload: Set your own schedule and visit cadence based on your personal availability and the patient's plan of care.
  • Document Efficiently: Use one simple EMR system for all notes (with tools available to minimize documentation time!).
What You'll Need
  • Valid Arizona professional license matching your discipline (PT, PTA, OT, or COTA)
  • CPR or BLS certification
  • Valid driver's license and auto insurance
  • Physical + TB test (within the last year)
  • Professional liability insurance and business license (if applicable)
