InfuCare Rx is a leading home infusion provider accredited by URAC and ACHC offering a specialized approach to caring for patients in need of IV therapy and specialty pharmacy services in the homecare setting. Description:

Reporting to the Pharmacist-In-Charge and Staff Pharmacists, the Pharmacy Service Representative will act as the patient’s advocate to ensure that the prescribed drug therapy, pharmacy assessment and pharmacy care plan are followed as prescribed and meets the medical, social, and personal needs of the patient.

Responsibilities:

  • Communicates with patients to ensure proper delivery of medications (both acute, IVIG and specialty) for scheduled infusion appointments.
  • During communication with patients, understand how to properly complete assessment.
  • Assists with determining patient availability for the Digital designation and will assist in education for the patient on using the Digital platform.
  • Ensures that patients receive notifications and understand appointment schedule.
  • Responds to patient calls timely and appropriately.
  • Obtains and updates necessary patient demographic and related data according to policies and procedures.
  • Data enters relevant communications in progress notes of pharmacy operating system.
  • Assists with picking and packing products in preparation for shipping as requested.
  • Attending meetings and other functions necessary to perform duties and assist in business. development for the Company, including assisting in launching a certain data collection software.
  • Perform other duties as assigned by management.
  • Proficient in computer program applications, knowledge, and skills.

Requirements:

  • Healthcare experience required
  • Must possess a current and in good standing Pharmacy Technician license in state of employment.
  • Specialty Pharmacy and Home Infusion Services experience preferred
  • Proficiency with Microsoft Office suite products
