Job description

Location/Department: Hospice
Part time - 16 hours per pay period
Job Summary:
The Hospice Community Liaison is responsible for developing, nurturing, and maintaining relationships with physicians, hospitals, skilled nursing facilities, and community organizations to increase awareness and appropriate utilization of Hospice services. This role serves as a trusted resource for healthcare professionals, patients, and families by educating them on hospice philosophy, eligibility, and benefits while representing our organization with compassion, professionalism, and integrity.
Supervisory Responsibilities: None
Duties/Responsibilities:
  • Cultivate strong relationships with referral sources to increase awareness of Hospice services and drive appropriate patient referrals.
  • Conduct regular in-person visits, calls, and meetings with community partners to deepen trust and identify new referral opportunities.
  • Educate healthcare professionals, patients, families, and community groups about hospice philosophy, the Medicare hospice benefit, and eligibility criteria in a clear and compassionate manner.
  • Represent MMH Hospice at health fairs, networking groups, and community events to increase awareness and visibility.
  • Collaborate closely with healthcare partners to ensure seamless referral transitions for both referring facilities and the families they serve.
  • Seek and incorporate feedback from referral partners and internal teams to continuously improve service and relationships.
  • Work closely with leadership to design and execute outreach programs and initiatives that align with organizational goals.
  • Developing and maintaining referral source and outcome tracking, analyzing trends, and regularly presenting data-driven insights and reports to leadership to inform strategy and performance improvement.
  • Uphold all compliance, ethical, and confidentiality standards.

Required Skills/Abilities:
  • Compassion, empathy, and emotional intelligence when interacting with patients, families, and healthcare staff.
  • Excellent verbal and written communication skills, including active listening.
  • Strong relationship-building and networking skills with physicians, facility staff, and community partners.
  • Problem-solving and critical thinking skills to remove barriers to referrals and address partner concerns.
  • Meticulous time-management and organizational skills.
  • Ability to track and analyze referral data and outcomes and translate findings into actionable strategies.
  • Self-directed and able to work independently.
  • Working knowledge of Hospice and Palliative Care services, eligibility criteria, and care processes.
  • Valid driver's license and willingness to travel.

Education and Experience:
  • Minimum Associate's Degree in Marketing, Business, Communications, Social Work, Nursing, or a related health or human services field; Bachelor's degree preferred or equivalent experience may be considered.
  • Approximately 2 years of successful sales, business development, or outreach experience, ideally in Hospice, Home Health, Palliative Care, Senior Care, Pharma, or other healthcare services.
  • Prior experience working in healthcare and/or with interdisciplinary care teams is highly preferred.
  • Proficient with Microsoft Office for tracking activity, referrals, and outcomes.

Physical Requirements:
  • Frequent use of hands, walking, sitting, standing, kneeling, reaching, stooping, talking.
  • Occasionally may have to lift over 50 lbs.
