Care Coordinator - HI ARNG
Kapolei, HI

Qualifications:
Medical Records or Health Information Specialist with 3 years of experience in a clinical setting OR 3 years of case management experience in an outpatient setting.
Utilization review or disease management experience or certification in case management is desirable. Proficient with the use of the following medical systems: 1) Medical Operational Data System (MODS) applications to include Medical Readiness Portal, MEDPROS Web Data Entry (MWDE) and Medical Data Protection System (MEDPROS); 2) Medical Electronic Data Care History And Readiness Tracking (MEDCHART) applications to include Dental Classification (DenClass), Electronic Case Management (eCase), Electronic Medical Management Processing System (eMMPS), and Health Readiness Records (HRR); 3) Armed Forces Health Longitudinal Technology (AHLTA); 4) HIARNG Medical Non-Deployable (MND) tracking system; and 5) the Warrior Transition Report (WTR)
Duties:
Maintain Soldier medical/dental records IAW Army Regulation (AR) 40-66. Follow the regulations and policies applicable to Individual Medical Readiness (IMR) and deployment, to include the AR 40- 501, AR 40-502, DA PAM 40-502, current USCENTCOM Individual Protection and Individual-Unit Deployment Policy, Internal State Surgeons Office (SSO) Standard Operating Procedures (SOP's) and internal guidelines and procedures.
Process and maintain automated referrals within the timeline indicated in MEDCHART.
Mark and safeguard files, output products, and storage media per classification level and disseminate them only to individuals authorized to receive them with a valid need to know.
Create, maintain, and process medical records for incoming, current, and exiting members within 14 business days of receipt of documents.
File documentation in individual medical/dental records in both the hard copy file and in the electronics Health Readiness Record (HRR) within 15 business days of receipt. Input into the medical record system documentation of results of Bi-monthly case reviews.
Perform quality assurance checks on in-coming medical/dental records. Track all discrepancies to closure. These checks need to be completed NLT 3 business days after receipt and discrepancies tracked to meet their suspense's.
Scrub electronic data system modules before medical/dental events to identify non- deployable Soldiers. Provide a list of non-deployable Soldiers to the COR NLT 5 days prior to event.
Contractor shall provide all administrative charting in eCase, DenClass, MEDPROS, Healthcare Portal, Medical Health Assessment/Periodic Health Assessment (MHA/PHA), MEDCHART, Automated Voucher System (AVS), AHLTA, MODS, and MWDE. Charting shall be completed NLT 3 business days after receipt of information.
Participate in case reviews to ensure that every Soldier with an open case is being tracked, managed, and communicated with at a minimum of every 30 calendar days.
Provide case processing, medical/dental documentation, and profile concerns to the care coordinator/dental care coordinator for review.
Provide medical administration services at MRE events as determined by the needs of the state.
Maintain Health Insurance Portability and Accountability Act of 1996 (HIPAA) standards and guidelines.
Review and track health assessment (PHA/SRP) lab results in orderto contact Service-Member (SM) and educate on lifestyle modifications.
Scan, upload, stamp and index medical documents of case managed SMs into HRR.
Collect all hard copies of civilian and military medical documentation to include printing ALTHA notes and ensuring they are scanned and indexed in HRR.
Establish and maintain bar code system to label all Soldier treatment records for all active Soldiers.
Enter immunizations, pregnancy, eye exams and medical warning tags into MODS within seven (7) business days of receipt.
Schedule:
Provide services between the hours of 0800 and 1630 Monday through Friday. Performance shall be at HIARNG Office of the State Surgeon (OTSS), 91-1227 Enterprise Blvd, Bldg. 46, Kapolei, HI
