Job description

Housing Stability Case Manager

Schedule: Monday through Friday, 8:00am-4:00pm

Position Summary
The Housing Stability Case Manager is responsible for the overall delivery of services and implementation of individualized case plans for domestic violence survivors following the Housing First approach to rapid re-housing. Services include safety planning, crisis intervention counseling, harm reduction, housing stabilization, job search, and maintaining permanent housing.

Responsibilities

  • Provide evidence-based case management services for up to 30 families with an emphasis on safety and housing first for survivors of domestic violence.
    •Complete initial intake and assessments of individuals to assess their needs.
    •Facilitate relationship building between landlords, residents and Homeless Services Network (HSN).
    •Assist residents in applying for and obtaining long term stable sources of income.
    •Follow Housing First methodology and provide mobile advocacy.
    •Utilize harm reduction interventions and methodologies to provide resident centered services that keep them in their housing.
    •Attend all trainings offered by HUD and HSN that increases knowledge of effectively meeting residents’ needs.
    •Work closely with other agency programs to maximize residents’ outcomes and program goals.
    •Maintain work schedule as required by program needs with adequate flexibility.
    •Document and maintain up to date information on services.
    •Ensure residents transition to permanent housing.
    •Ensure residents gain earned income/increase income while in the program.
    •Arrange and facilitate monthly resident’s meetings/outings.
    •Conduct weekly home visits and/or appointments with residents to review progress toward goals and self-sufficiency.
    •Perform all other duties as assigned.

Qualifications

  • Applicant must have a bachelor's degree in social work or in related field.
  • Two years' experience working with diverse populations and domestic violence survivors preferred.
  • Rapid Rehousing experience preferred.
  • Experience in counseling, crisis intervention, problem solving, case management, program planning, and time management.
  • Applicants must be able to work independently and provide services to clients with excellence
  • A current valid Florida driver’s license is required for this position
  • Current personal auto insurance with minimum financial responsibility and no-fault coverage required by law is mandatory
