Summary
The RN, Nurse Navigator utilizes a collaborative process to assess, plan, implement, facilitate, and monitor patients along their clinical pathways. The RN, Nurse Navigator works closely with the patient and family, making appropriate referrals to meet the individual, cultural, and spiritual needs of the patient. The RN, Nurse Navigator monitors and evaluates the services provided to the patient.
Essential Duties and Responsibilities
These duties and responsibilities described below represent the general tasks performed on a daily basis. Other duties may be assigned as needed to support the vision, fulfill the mission, and uphold the values of the organization.
  • Attend required hospital-wide orientations, meetings, and in-services
  • Demonstrate a commitment to flexible work scheduling when necessary to ensure patient care
  • Perform other duties as assigned
  • Demonstrate an understanding of Cancer Services management philosophy and principles, and the patient/family orientation to their specific program
  • Coordinate procedures and evaluations to meet the standard of care established by the American Cancer Society and related subcommittees
  • Identify, educate, and guide patients with a cancer diagnosis or abnormal finding through clinical pathways in collaboration with physicians and the healthcare team
  • Identify patients' physical, psychosocial, spiritual, and cultural needs through ongoing assessment; make appropriate referrals to the cancer healthcare team and community resources
  • Develop an individualized plan of care for each patient/family based on assessed needs
  • Communicate pertinent clinical information to appropriate members of the healthcare team regarding patient/family desired outcomes and treatment choices
  • Establish and maintain positive relationships with community and referring physicians
  • Maintain ongoing communication among patient/family, providers, and sites of care to ensure continuity and coordination of care
  • Work collaboratively with Chesapeake Regional Healthcare departments to meet program and health system goals
  • Document all activities and decisions related to patient care in compliance with state and national regulatory standards
  • Gather quality and performance improvement data for the program steering committee and the Cancer Committee as requested
  • Advocate for the highest level of customer service; ensure the center meets or exceeds internal and external customer expectations
  • Participate in strategic planning
  • Provide education and support to patients/families through support groups, educational materials, individual teaching, programs, and community outreach
