Westside Community Focus Inc. is a non-profit organization dedicated to teaching independent living skills, as well as providing housing, advocacy and other services that primarily assist adults who have developmental disabilities. We strive to promote the confidence and capability of our clients, encouraging the acquisition and maintenance of valuable life skills necessary to live successfully and productively in the community.

We have a large Independent Living Program and three Group Homes. We are seeking the following: - Direct Care staff for our SE Group Home Program- 1 Full time swing Thursday- Monday: 2pm- 10pm Occasional overtime may be required We are searching for individuals who are excited to help make a difference in the lives of others. Our Direct Care staff work one-on-one with clients in a group home setting, helping to provide care and assistance with client identified goals; light housekeeping and cooking; companionship and community inclusion; med administration and hygiene assistance; errands, attending appointments, and more.

Duties and Responsibilities include the following . Other duties may be assigned.1. Supervise client in matter concerning their health and safety

*2. Accompany clients in the community via tri-met. *3.

Attend medical appointments with clients and provide physicians with necessary documentation. *4. Monitor medication administration, including documentation of medication errors*.5

Prepare meals and assure food is properly stored. *6. Maintain accurate performance data (client log or specific data tracking via Therap) and assist in completion of ISP goals for client.

*7. Conduct formal and informal training with residents to help them acquire living skills that can lead to increased independence. *8.

Submit written Incident Reports of any accidents, injuries or serious problems involving the residents (by the end of your shift). Report any suspected abuse per the abuse reporting procedures as a mandatory reporter. *9.

Attend and participate in staff meetings and trainings. *10. Monitor the appearance of client and offer assistance in personal hygiene issues.

Document in the Direct Care T-log or on tracking sheet any specific areas of concern regarding hygiene issues. *11. Encourage and assist client in the use of their leisure and recreational time.

*12. Encourage client to keep his/her room orderly and sanitary. *13.

Help client protect his/her personal property and learn to care for personal property. *14. Record significant occurrences or information that should be shared with other staff in the Direct CareT- log or on tracking sheets.

*15. Help client maintain his/her personal schedule. *16.

Model and encourage appropriate socialization skills. *17. Maintain scheduled work shifts and trainings.

Work Environment: Direct Care Staff work with various clients in a Westside Community Focus Group Home and may also take the client out to medical appointments, recreational outings, grocery shopping, etc. Direct Care Staff are exposed to various situations with varying degrees of cleanliness.

There is potential exposure to body fluids. Some clients have pets; there is exposure to cats and litter boxes in some homes. Our clients often have difficult, unpredictable personalities; there is the potential to be exposed to verbal and/or physical aggression from the client.

Physical Demands: The physical demands of the job vary depending on the client and the needs of the client and vary with each visit to a particular client. In general, Direct Care Staff lift up to 25 pounds 0-10% of the time and five to ten pounds 0-25%. They use hands on a daily basis, reach 0-25% of the time and depending on the day, stand, walk and/or sit.

They will have to climb stairs when arriving and leaving a client's apartment and are required to talk and hear 100% of the time. We will provide on the job training.
