Position Overview:
Job Title: Direct Support Professional - Per Diem I
Location: Rochester, NY 14624 - Needed specifically in Spencerport
Department: Relief
Reports To: Workforce Management
Employment Type: Per Diem - Minimum of 16 Hours every 2 weeks required
Starting Wage: $17.44/hr (Day Services) | $18.47/hr (Residential)

Your Core Responsibilities:
  • Provide respectful personal care including bathing, grooming, toileting, dressing, and meal support.
  • Support individual goals, independence, and skill development while promoting community inclusion.
  • Implement individualized plans including IPOP, SAP, BSP, and PONS.
  • Maintain accurate documentation in the Electronic Health Record (EHR).
  • Ensure health, safety, dignity, and well-being at all times, reporting concerns immediately.
  • Foster positive communication with individuals, families, co-workers, and supervisors.
  • Drive agency vehicles to transport individuals to appointments, programs, or community outings.

What You Bring:
  • Compassion, patience, dependability, and respect for others.
  • Ability to follow individualized care plans and maintain required supervision levels.
  • Basic computer skills for documentation and communication.
  • Strong communication and problem-solving abilities.
  • Flexibility to work at various sites and shifts as needed.
  • Ability to lift/move up to 35 pounds and perform physical care responsibilities.
  • Required: Maintain certifications in SCIP-R and First Aid/CPR (training provided).
  • Must pass required background checks and clearances (SCR, fingerprinting, SEL, MHL).
  • Preferred: Prior experience in human services or direct care.
