Registered Nurse (RN) – Home Health

Job Title: Registered Nurse (RN) – Home Health
Department: Clinical Services
Reports To: Director of Patient Care Services (DPCS)/Director of Nursing (DON)
Employment Type: Full-Time / Part-Time / Per Diem

Position Summary

Heroes Healthcare is seeking an experienced and compassionate Registered Nurse (RN) to provide skilled nursing care to patients in their homes. The ideal candidate must have prior home health experience and be knowledgeable in Medicare Conditions of Participation (CoPs), OASIS documentation, care coordination, wound care, disease management, and interdisciplinary collaboration.

This position requires independent clinical judgment, exceptional documentation skills, and the ability to manage a caseload while ensuring compliance with all federal, state, and agency regulations.

Mandatory Qualifications

Applicants who do not meet the following requirements will not be considered.

  • Current California Registered Nurse (RN) license in good standing.
  • Minimum of one (1) year of recent Home Health Registered Nurse experience (REQUIRED).
  • Minimum of one (1) year of OASIS documentation experience (REQUIRED).
  • Current BLS/CPR certification.
  • Valid California driver’s license.
  • Reliable insured vehicle for travel between patient homes.
  • Ability to pass background check, health screening, and drug screening.

Preferred Qualifications

  • Experience with WellSky/Kinnser or similar EMR.
  • Wound Care Certification (preferred).
  • IV infusion experience.
  • PICC line management.
  • Foley catheter management.
  • Hospice experience is a plus.

Essential Duties and Responsibilities

  • Complete Start of Care (SOC), Resumption of Care (ROC), Recertification, Follow-Up, and Discharge visits.
  • Complete accurate and timely OASIS assessments.
  • Develop and update individualized plans of care.
  • Perform comprehensive nursing assessments.
  • Administer medications and treatments.
  • Provide wound care using evidence-based practices.
  • Manage IV therapy, central lines, PICC lines, ports, and infusion medications.
  • Perform catheter care and other skilled nursing procedures.
  • Educate patients and caregivers on disease management, medications, safety, and infection prevention.
  • Monitor patient progress and promptly report significant changes to physicians.
  • Coordinate care with Physical Therapy, Occupational Therapy, Speech Therapy, Medical Social Worker, Home Health Aide, physicians, case managers, and referral sources.
  • Maintain compliance with Medicare, state, and agency regulations.
  • Participate in case conferences and interdisciplinary meetings.
  • Ensure patient rights, confidentiality, and HIPAA compliance.

Documentation Requirements

  • Complete all clinical documentation accurately and within agency timeframes.
  • Complete Start of Care documentation within required agency deadlines.
  • Submit visit notes promptly after each patient visit.
  • Upload wound photographs and measurements as required.
  • Maintain accurate medication reconciliation.
  • Document physician communications and new orders timely.
  • Complete all documentation in accordance with Medicare regulations.
